STEP 2. Then print out the 2 forms:
The Trail Blazer Code and The Club Blazer Sign-up Form.

STEP 3. Carefully read over The Trail Blazer Code and sign your name at the bottom. Then fill in the contact information and answer the questions on The Club Blazer Sign-up Form.

Mail the completed forms to:

Once we've processed your membership form we'll send you:
